Athlete: I hereby acknowledge and state that:

1. I, as a participant, will use every effort to be a positive member of Play Higher FC programs and exhibit good moral behavior and sportsmanship at all times.
2. will keep in mind that exhibiting good moral behavior and sportsmanship includes being helpful to other Play Higher members and staff, no swearing or using derogatory
   language, no bullying or criticizing others, no stealing, and other negative actions directed towards Play Higher FC members and staff.
3. I will be responsive to my coaches and teammates.
4. I will keep my grades at an appropriate level to participate in Play Higher. If I am having problems at school, I will ask Play Higher FC staff for help.
5. I will not attempt ever to deliberately injure another player.
6. I will conduct myself in a respectable manner at all Play Higher FC sessions, social media interactions, and school events.
7. I realize that failure to comply with the above Code of Conduct will result in a loss of privileges or suspension from Play Higher FC.
